Moving into adulthood
For young people with a learning disability, moving from children's to adult services can be a significant change.
Transition planning supports young people and their families to think about future options, such as:
- learning and education
- employment or training
- where and how to live
- health and wellbeing
- building independence and life skills

- discussing goals and aspirations

During times of change, people may benefit from additional or temporary support.
This could include:
- extra guidance or coordination
- practical help at home
- support with decision‑making or communication
- working with more than one service at the same time
