The Perth and Kinross Health and Social Care Partnership is a collaboration between Perth and Kinross Council, NHS Tayside, and a broad network of Third Sector, Independent Sector, and community partners.
By working together, we deliver joined‑up health and social care services that support adults across Perth and Kinross to live well, independently, and safely at home for as long as possible.
Every day, our teams work with people who are ill, vulnerable, or living with disabilities, helping them access the right support at the right time. Our approach is centred on prevention, early intervention, and providing care in ways that are simple, coordinated, and person‑centred.

Our history
The HSCP was created in response to the Scottish Government's commitment to delivering more integrated health and social care. Since its formation, the Partnership has grown to include a wide range of organisations working together to improve outcomes for adults across the region.
This integrated model helps us:
- prevent unnecessary hospital admissions
- support smoother transitions between services
- reduce delays in care
- provide community‑based support that keeps people connected to their lives
Over time, our collaborative approach has strengthened relationships with local communities, practitioners, carers, and organisations, ensuring services are designed and delivered with the people who use them.
Our leadership
The Partnership is led by a senior team responsible for strategic planning, operational delivery, clinical governance, and service improvement across Perth and Kinross.

Head of Integrated Health and Care (Adults) - Jane Menzies
Responsible for services relating to:
- learning disabilities
- mental health and wellbeing
- physical disabilities
- addictions
- employability support
Our strategic priorities
Over the next three years, the Partnership will focus on seven key priorities designed to improve health and wellbeing outcomes across Perth and Kinross:

The Integration Joint Board (IJB)
The Integration Joint Board (IJB) is responsible for the strategic planning, oversight, and delivery of integrated adult health and social care services in Perth and Kinross. It brings together:
- Elected members
- NHS non‑executive members
- Representatives from the Third Sector
- Senior officers from NHS Tayside and Perth & Kinross Council
- Unpaid carers and people with lived experience
The IJB was formally established in October 2015, marking a significant step in how health and social care is designed and delivered across the region.
