Referral process

How to refer someone for care and support - or make a referral on their behalf

Our aim is to make it as simple as possible for you to let us know when you or someone you care for needs support. Most referrals in Perth & Kinross now start with our Early Intervention & Prevention Team, who will help you explore options, understand what you need, and connect you with the right support at the right time.

Who can make a referral?

You can make a referral if you are:

  • an adult needing support
  • a family member or carer
  • a friend or neighbour with concerns
  • a GP, nurse, housing officer, voluntary organisation, or other professional

You do not need a diagnosis, formal paperwork, or a GP appointment before contacting us.

Step 1 - Contact the Early Intervention & Prevention Team

This is the first point of contact for all new enquiries about care, support, or community‑based help.

The team will:

  • listen to your situation
  • help identify what support might help
  • offer advice and early intervention options
  • arrange an assessment if needed

Step 2 - Sharing information

When you get in touch, we will ask for:

  • your name and contact details
  • information about your needs or concerns
  • any risks or urgent issues
  • relevant medical, social, or living‑arrangement details
  • details of other professionals involved (if any)

If you are making a referral on someone else's behalf, we encourage you to have their consent whenever possible.
If this isn't possible due to urgency or concerns about safety, we will still listen and advise.

Step 3 - Screening and triage

The Early Intervention & Prevention Team will review the information and decide the most appropriate next steps, which may include:

  • advice, guidance, or signposting
  • community‑based services
  • wellbeing support
  • carer support
  • equipment or adaptations
  • arranging a social work assessment

If there is immediate risk or a safety concern, we may prioritise the case straight away.

Step 4 - Assessment (if required)

If an assessment is needed, you will be referred to the most appropriate team.
A worker will contact you to arrange a visit or conversation at a time that works for you.

The assessment helps us understand:

  • your strengths
  • what matters most to you
  • where you may need support
  • how we can help maintain your independence and wellbeing

 

Step 5 - Planning your support

If you are eligible for support, we will work with you to:

  • agree outcomes that matter to you
  • explore options and choices
  • create a personalised support plan
  • arrange any services or community connections

If you are not eligible, we will always provide advice and help you find other support.

Step 6 - If your situation changes

You can contact us again at any time if:

  • your needs increase
  • something changes at home
  • you are struggling with your current support
