Applying for care and support is simple. Whether you need help at home, support after an illness, advice for a family member, or access to specialist services, we're here to guide you through every step.
Step 1 - Contact the Early Intervention and Prevention Team (EIPT)
Our 'Early Intervention and Prevention Team' is the first point of contact for adults (16+) who need access to health and social care support in Perth and Kinross.
A member of the team will talk with you about your situation, offer initial advice, and help you begin the assessment process.

What the team can help with
The Early Intervention and Prevention Team offers advice and information on:
- support for carers (including respite and day opportunities)
- care at home and reablement services
- meal services
- Community Alarm and Telecare
- access to residential or supported accommodation
- services for learning disability, mental health or substance use
- welfare rights, benefits and the Scottish Welfare Fund
- referrals for Social Work support
- adult support and protection concerns
They can also connect you with local GPs, third sector partners, and community organisations.
Short‑term support (up to 4 weeks)
For new clients, the team may offer up to 4 weeks of early intervention support to help you live more independently at home and in your community.
This may include:
- reablement support
- equipment or technology
- short‑term home care
- community‑based support
- practical advice and planning
If your needs are more complex and support is required beyond 4 weeks, you will be transferred to a key worker in your local area, who will continue working with you to achieve your goals.
If you are already receiving support
If you already have a social care support package in place, please contact your existing key worker.
They will help with changes, reviews or any concerns.
Step 2 - Your assessment
After you contact EIPT, we will arrange an assessment to understand your needs. Assessments are always free, confidential, and carried out by trained professionals.
Assessments may take place:
- in your home
- by phone
- via video call
- during a hospital stay (if relevant)
During your assessment, we will:
- talk about what matters most to you
- explore what you're finding difficult
- understand what's working well
- look at any support you already have
- discuss equipment, adaptations or specialist services
- explain your options
- agree on next steps
We want to make sure the support you receive fits your needs, goals and circumstances.
Step 3 - Your personalised care plan
After the assessment, we'll work with you (and your family or carer if you choose) to create a personalised care plan.
Your plan might include:
- home care or personal support
- reablement
- equipment or home adaptations
- community groups or day services
- mental health or wellbeing support
- support for carers
- technology enabled care / telecare
- specialist health services
- advice, information or signposting
- referral to another team or partner organisation
We'll explain every option clearly so you can make the choices that feel right for you.
